To request items, you may fax or mail the order form to our office.  Although these items are offered free of charge, there is a cost to produce and ship them.  Your donation to help cover these costs is greatly appreciated; the cost of these items is provided on the order form as a guideline.  For donations over $10.00 at tax receipt will be issued.
